Who is responsible for your information?
CRR Kambô is operated by Christopher Rowan in Dunbar, Scotland. I am responsible for the personal information you provide through this website and during enquiries, health screening and Kambô sessions.
Information I may collect
This may include your name and contact details, information you provide when making an enquiry, and health and safety information supplied through the Health & Safety Screening Questionnaire. Health information is treated as sensitive personal information and is handled with particular care.
Why I use your information
Your information is used only where necessary to respond to enquiries, carry out health and safety screening, discuss suitability and preparation for a possible Kambô ceremony, communicate with you about a booking, and maintain appropriate records. Where consent is relied upon for sensitive health information, you may withdraw that consent, although this may mean I cannot safely offer a ceremony.
Health questionnaire and Jotform
The Health & Safety Screening Questionnaire is submitted using Jotform's encrypted form system. Completed questionnaires are stored as encrypted submissions and are accessed through the protected CRR Kambô Jotform account using the encryption access code. Jotform acts as a service provider used to collect and store these submissions.
Sharing your information
I do not sell your personal information. Information will not normally be shared with other people or organisations except where this is necessary to provide a service you have requested, where you have agreed to it, or where disclosure is required by law or necessary to protect someone's vital interests.
How long your information is kept
Health screening questionnaires and information relating to your Kambô ceremony will normally be retained securely for three years, after which they will be securely deleted unless there is a legitimate reason or legal requirement to retain them for longer. General enquiries that do not lead to a ceremony will not be kept longer than reasonably necessary.
Your rights
Under UK data protection law, you may have rights over your personal information, including rights to ask for access to it, correction of inaccurate information, deletion or restriction in certain circumstances, and to object to certain uses. You may also withdraw consent where consent is the basis on which information is being used.
Questions or requests
If you have a question about your information or want to exercise a data protection right, please contact Christopher Rowan at crrkambo@gmail.com.
If you are unhappy with how your personal information has been handled, you also have the right to raise a concern with the UK Information Commissioner's Office (ICO).
